School of Nursing:

If you have attended Ohio University at any point in the past (even if it was long ago), you will need to complete an Update of Program Form available from the School of Nursing or any of the Ohio University branch campuses. This indicates that you wish to change your major to the Bachelor of Science in Nursing. You may obtain this form by contacting the School of Nursing by any of the means listed below. Once completed, mail this form to the School of Nursing.

You also need to complete an Inquiry Form. This form is accessible in our right side download menu at the very bottom. Or you can contact the School of Nursing and we will send one to you (via email), but the best way to complete this form is to call the School of Nursing (740-593-4494) and we can help answer and address any other quesitons.

If you have attended any colleges or universities other than Ohio University, you will need to mail or fax a copy of those transcripts to the School of Nursing.

Office of Admissions:

In addition to contacting the School of Nursing, you must arrange to have official copies of your transcripts sent directly to the Office of Admissions at one of three regional campuses:

The following deadlines pertain to any quarter in which you you may be seeking admission. For example, if you wish to attend in Fall Quarter, you need to submit your application by May 15, and your transcripts must be received by June 15.

Requirements

In addition to completing the application process, you, as a prospective student who wishes to obtain the Bachelor of Science in Nursing degree from Ohio University, must meet the following criteria:

Demonstrate you are a graduate of a state board of nursing approved associate degree or diploma program in nursing.

Obtain admission to Ohio University.

Undergo an evaluation by Ohio University and the School of Nursing of all of your transcripts from all the schools you attended since high school and/or the evaluation of scores from all proficiency or professional tests you have taken.

Demonstrate that you hold a current license to practice as a registered nurse.

Provide documentation that your immunizations (tetanus, MMR, polio, and Hepatitis B) are current or provide appropriate waivers.

Provide documentation of your annual Mantoux testing for TB and any prescribed follow-up (or provide appropriate waivers).

Provide documentation that you are currently certificatied to perform CPR.

Provide documentation that you are currently covered by malpractice insurance.
